I understand that any insurance terms and definitions are meant for general informational purposes and not meant as specific advice or recommendations. For detailed information, it is my responsibility to contact Mid-Valley Insurance.

I understand that no coverage will be assumed bound or covered until I have received written or verbal confirmation from a licensed agent of Mid-Valley Insurance.

I understand that Mid-Valley Insurance will receive documentation of service requests, requests for information or declination for additional information, which may become part of my customer file.

I understand that Mid-Valley Insurance will process service requests based on information provided through this site and that I will receive confirmation of all changes to my coverage. I authorize all such changes, and assume the responsibility of notifying Mid-Valley Insurance in the event of errors or misunderstandings.

1. Guarantee of privacy of client information and all privacy language.

2. Mid-Valley Insurance will handle service requests in good faith based on registration information. All service requests will be confirmed to the client and they must contact Mid-Valley Insurance in case of error.

3. You are responsible for updating email address and other information in the event of change.

4. Insurance terms, descriptions of coverage and other insurance information is not meant to be complete, and the client should either contact Mid-Valley Insurance or read their policy for complete information.

5. All insurance and non-insurance information is for general purposes and may not be up-to-date or complete. Mid-Valley Insurance is not responsible for information in sites linked.

6. No changes will be considered to be bound until the client has received confirmation from Mid-Valley Insurance.

7. You are solely responsible for all security issues surrounding use of their private password. Mid-Valley Insurance cannot access password therefore client acknowledges and accepts full responsibility if password is given to another party.

When you enroll for an insurance policy at Mid-Valley Insurance, we will ask you to provide your name, home address, mailing address, telephone number and e-mail address. This information will be provided to the insurance company when you purchase an insurance policy so they can establish you as a policyholder. We will maintain a record of your information at the offices of Mid-Valley Insurance. so we can provide you with policyholder service. The employees of Mid-Valley Insurance are each required to sign and acknowledge a Confidentiality And Nondisclosure Agreement. Each employee has been instructed on maintaining the privacy of each customer and the importance of protecting the customer's personal information.

 

When you purchase an insurance policy from Mid-Valley Insurance, you will pay the premiums with your credit card or personal check. If you use your credit card to pay the premium, your credit card information is deleted from our records after your purchase has been authorized. A transaction number will be provided to you on your Binder Of Insurance. You may use this transaction number as a reference to the credit card purchase. We do not keep a record of your credit card information. If you pay the premium using your personal check, we will keep a copy of your check in your file.

 

When you send us e-mail from this Web site, you will provide us with certain personally identifiable information including your e-mail address.

 

Cookies

A cookie is a piece of data that is stored on a visitor's hard drive while they are visiting this Web site. We may utilize a short-lived form of cookie when you visit this site.
